Icon file does not work...

i need some help guys... as the subject says... the icon file does not work...

I tried putting an image on the icon file but it doesn't seem to work. any ideas on why this does not work?
i've tried saving it on every file extension but doesn't work.. does it need to be .ico to work?

It does need to be a .ico file. There should be several topics explaining the proper size and similar. The ico files produced by this site are said to work.
